Investigation Summary
On April 14, 1999, Employee #1 was operating a metal-cutting band saw (CMI-Serial Number 438-871769, Model C-916), cutting a bundle of thirty-six-pieces of U-shaped metal channels (approximately 3 in. by 1.5 in. by 8 ft) that was wrapped together with a metal strap. Employee #1 was operating the machine and started clearing away the metal shavings using a spatula as reported by a witness. Employee #1's glove got caught in the running blade and his hand was pulled into the saw blade. Employee #1 suffered a deep laceration to his hand and was hospitalized.
