Employee fractures leg in fall from drill machine pipes

At approximately 1:15 p.m. on August 27, 1998, Employee #1, of Votaw Precision Technologies, was standing on the pipes of a DF Clevis drill machine while hooking a crane to an overhead ceiling panel. He had his legs apart when he lost his footing and fell 42 in. to the floor, fracturing his right leg. A safe platform was not provided from which to perform the work. Employee #1 later stated that this work practice had been used in the past without any accidents.
