Investigation Summary
On June 27, 1997, Employee #1, a sales manager, was walking from his office to the freezer department, where food was stored and shipped out on a daily basis. On the way back to his office he slipped and fell, pulling a muscle in his back. He was treated at the hospital and then told to rest. Employee #1 had been wearing street shoes, not rubber boots.
