Investigation Summary
On November 9, 1996, Employee #1 and a coworker, both of whom had been leased to the Louisville Ladder Corporation, were working under the control of the Louisville managerial staff. At approximately 12:05 p.m., Employee #1 was standing on a wooden pallet on the elevated forks of a forklift, fixing a sign on the wall of the dock area. The forklift tipped to one side and Employee #1 fell between 15 and 20 ft, fracturing his right foot. The pallet was not secured, and Employee #1 was not tied-off.
