Investigation Summary
At approximately 4:00 p.m. on November 12, 1996, Employee #1 was working in the packaging area of Sunrider Manufacturing. With her right hand, she was adjusting a box that had become stuck inside a shrink-wrap machine. She was using her left hand to press the manual feed button when she was struck by the reeling bar. Employee #1 suffered a laceration and partial amputation of her right middle finger and was taken to a clinic in West Covina, where she was treated and released the same day. She reported back to work on November 14, 1997. There were no direct witnesses to this accident.
