Investigation Summary
At approximately 8:30 a.m. on September 21, 1999, Employee #1 was operating Extruding machine Number 2 extruding 1/2 in. thick round shaped wax parts used in foundries for casting. The front plate of the die fell off the frame and his left index finger was caught between the plate and a sharp edge of the table resulting in amputation of his left index finger. He was hospitalized.
