Investigation Summary
At approximately 6:00 p.m. on March 20, 1997, Employee #1, of the Organic Milling Company, was clearing some jammed flour from an extruder machine in the production department. He was reaching into the extruder when his left hand struck the moving parts of the blender, amputating a portion of his third and fourth fingers. Employee #1 later stated that he thought the power to the machine was off because the green lamp on the power control panel was not glowing.
