At approximately 9:00 a.m. on August 21, 1997, Employee #1, a contract worker with the California Staffing Corporation, was employed at a company that manufactured cloth and dyes. She was walking out of the lunchroom when she slipped, tried unsuccessfully to regain her balance, and then fell to the ground. Employee #1 suffered a broken ankle.
