Employee's finger amputated in pulley assembly

Employee #1 was working on the belt and pulley assembly of a labeling machine when the belt caught his left index finger and pulled it up into the top pulley assembly. A portion of his finger was amputated. The machine had been turned off, but the conveyor was still in motion when Employee #1 first pulled on the belt with his finger.
