Employee's finger amputated in forklift mast chain

At approximately 6:15 p.m. on July 23, 1997, Employee #1, of ConWay Western Express, was standing on a wooden pallet that was being lowered by a forklift inside a delivery truck in the warehouse dock area. He was holding onto the forklift mast to maintain his balance when his right index finger became caught in the roller chain of the mast and was amputated. He was hospitalized for several hours, then released to recuperate at home.
