Investigation Summary
At approximately 2:30 p.m. on September 5, 1996, Employee #1, of Dekker's Welding Service, was working at a tank farm. He was walking along a pipeline to fix a valve when he fell approximately 1 ft off the pipe and into 180 degree F. water that had flooded the floor from a ruptured storage tank. Some of the scalding water entered his rubber boots, resulting in second-degree burns on his legs and feet.
