At 5:40 p.m. on October 31, 1997, Employee #1, of Consolidated Converter Company, Whittier, CA, was operating a machine that made toilet seat covers when the machine jammed, causing a stoppage. He tried to remove a piece of stuck paper from the machine while it was running and suffered an avulsion of his left thumb and almost complete amputation of the distal phalange.
