Investigation Summary
At approximately 2:15 p.m. on November 4, 1996, Employees #1 and #2, of the Universal Painting Company, were painting from a scissor lift. They were moving the lift to one side along the wall they were painting when it tipped over and they fell about 15 to 20 ft. Employees #1 and #2 were flown by helicopter to USC Medial Center, where they were hospitalized for a week or two and then released to recuperate at a convalescent hospital. In a subsequent interview, Employee #1 stated that the floor on which they were moving the scissor lift had a sloped surface that he could not see from the top and this caused the lift to overturn. Employee #2 could not be reached for an interview.
