Investigation Summary

Investigation Nr: 170899835
Event: 10/30/1996
Employee burned when splashed by sulfuric acid

At approximately 7:30 a.m. on October 30, 1996, Employee #1 was cleaning up water from a roof leak and removing boxes from a shelf under the leak. He was pulling a box off the shelf when it slipped and fell to the floor. The contents of two of the eight 1-gallon plastic containers spilled out and splashed Employee #1, causing minor burns on his neck and leg,. He was taken to Good Samaritan Hospital, Los Angeles, CA, where he was treated and released the same day. The plastic containers were labeled as corrosive and were determined to contain 98% sulfuric acid. Causal factors in this accident include the supervisor not identifying the chemical (sulfuric acid) inside the box, and Employee #1 not being trained to work around chemical hazards.
