Investigation Summary
At approximately 10:30 a.m. on February 29, 2004, Employee #1 was moving a 05 ft by 4 in.by 8 in. plexiglass around the perimeter of the arena set up for a hockey game. These are spread around to replace any glass that may break in the course of the game. According to Employee #1, there were about 15 panels on the glass rack. He was standing by the side when he pulled a panel. He was assigned to set two panels; he already pulled one and was pulling the second when the entire rack tipped on its side and dropped on him. Each panel weighs about 60 pounds. Employee #1 sustained a broken right leg and was hospitalized for 17 days.
