Investigation Summary

Investigation Nr: 201162385
Event: 11/28/2000
Employee Injured Using Roller Machine

Employee #1 was employed as a mixer machine operator. On November 28, 2000, Employee #1 was training a coworker on how to operate the machinery in the cup noodle department. One piece of machinery, the Menki Series Roller machine has a series of five Teflon coated rollers and one stainless roller. The dough sheet is fed manually on some occasions, depending on the product, into the machine's rollers. The Menki rollers gradually roll the dough into a flat sheet. The Menki rollers have emergency stop buttons located at each roller. Each set of rollers on the Menki machine is equipped with a safety guard that covers the rollers when the machine is in operation. The main function of the machine operator is to make the necessary adjustments to the rollers when needed, by turning the adjustments knobs located on the side of the machine. Just before the accident another coworker observed Employee #1 the first coworker standing and observing the machine and noted no problems. Employee #1 and the first coworker were listening to a noise coming from roller Number 2, when they noticed that the dough was moving to the side because some dough was sticking at the top of the roller. Employee #1 attempted to remove the dough with a scraper, while the rollers were still in motion and his hand was pulled into the machine. Employee #1 suffered fractures of the thumb and was hospitalized.
