Investigation Summary
At approximately 1:00 p.m. on July 1, 1999, two employees were working in a 3.5 ft deep section of a trench. The employees were gluing together a 5-in. PVC pipe inside the trench. Employee #1 was bending down while working inside the trench. A mound of dirt fell and pinned Employee #1 up to his chest. He was uncovered by coworkers. Employee #1 was hospitalized at County USC Hospital.
