Employee #1 was hospitalized with fracture, laceration.

At approximately 8:45 a.m., on February 7, 2001, Employee #1 was seriously injured while engaged in the process of rabbeting drawer parts using Table Saw, set up with 3/8 in. dado blade, the drawer part slipped into the throat opening on the table and kicked back striking Employee #1 on his right palm, and wrist area and then on the bottom of his chin. This equipment is also equipped with a rolling table which can be moved 4 in. away from the saw to permit installation of dado saw blades and other special heads for dadoing, rabbeting or grooving. Employee #1 was moving the scrap piece out of the way and about to set the piece on the completed pile, after completing the rabbet joint, the piece slipped out Employee #1's right hand and fell into the back throat opening approximately 5/8 in. by 10 in. The piece then got sucked into the rapidly moving saw blade and kicked back and hit Employee #1 on his right palm, and wrist and then hit lower part of his chin. As a result of this accident, Employee #1 had an incurred chin laceration and contusion, teeth avulsion, right wrist contusion/ sprain. The accident, was due to not using an Anti-kickback" device while rabbeting. Employee #1 was hospitalized with fracture.
