Investigation Summary

On October 12, 1999, Employee #1, Purchasing Manager for Lexington Scenery & Prop Manufacturer, Sun Valley, CA 91352. It was Employee #1's first day back to work from a non-industrial foot surgery. Employee #1 was on crutches when he went upstairs to the second floor Accounting Department to resolve a payroll issue. On his way back downstairs his left crutch apparently got caught on one of the steps causing Employee #1 to lose his balance and fall down approximately 20 ft to the bottom of the stairs. He sustain bruises and was hospitalized.
