Investigation Summary
At approximately 7:15 a.m. on July 16, 2001, Employee #1, a set painter since 1993 and a laborer for motion picture television program company, was painting a 4-foot high header with a telescoping roller. The employee was standing on the second floor catwalk, measuring 3 feet, 5 inches wide. The catwalk was located approximately 12 feet above from the stage floor. On the day of the incident, Employee #1 began working at approximately 6:00 a.m. under the direction of a painting foreman. Employee #1 was putting the final coat on the wall and the header on the second floor. Employee #1 took all the relevant materials with her to the second floor. Another worker helped Employee #1 masked off the areas to be painted. After all the masking was done, Employee #1 started to paint by herself. She started out with the wall area and proceeded to the header part facing the center lobby area. The bottom of the header was approximately 11 feet from the second floor catwalk and Employee #1 used a telescoping roller brush to reach this area while standing on the catwalk. Immediately behind the catwalk was a store front wall with openings measuring 15 feet, 8 inches wide and 11 feet high, ready for window and door installation. While painting, Employee #1 accidentally stepped back and fell through the unguarded wall opening onto the plywood below, landing on her right side. After the fall, other people nearby rushed to provide assistance, calling 911. A studio nurse provided assistance until the Culver City paramedics arrived and transported Employee #1 to the UCLA Medical Center. Employee #1 fractured 11 ribs, her right fibula, right great toe, and neck area and suffered lacerations to the scalp. She was hospitalized for her injuries.
