Event: 05/24/2005
Employee Is Injured in Fall Through Floor

On May 24, 2005, Employee #1 was instructed by his supervisor to clean and remove the debris from an apartment unit located on the second floor. As Employee #1 was sweeping the floor, he saw a piece of plywood lying on the floor. In order to continue his work, he lifted the plywood and leaned it towards the wall. He then took a step, not realizing that there was an opening approximately 43 inches by 43 inches where the board once was, and fell through the opening and landed on the floor below. Employee #1 was hospitalized at USC Medical center and treated for an injured right knee, requiring surgery.
