Investigation Summary
At approximately 6:27 p.m. on October 19, 2004, Employee #1 and a coworker were lifted up on the roof of a cage about 10 ft high from the ground by riding on the forks of a forklift. They were thrown from the forks when the forklift backed up. Employee #1 was hospitalized with a broken leg and shattered ankle.
