Employee Injured In Fall From Ladder

At approximately 7:30 a.m. on March 14, 2001, there were approximately five employees working on-site. Employee #1 was the job superintendent at the site. His supervisor was not at the job site. At the time of accident, the employees were breaking concrete at the basement for footings of new building and also removing heating and air pump and other equipment in basement. Employee #1 was operating a heavy equipment to load broken concrete in a truck. He decided to go down the basement using the ladder that was already sitting on there to check on something. The ladder was aluminum and approximately 12 ft. long. The distance from floor level down to basement was approximately 11 ft. Employee #1 stepped on the ladder and when he was on the second rung the ladder started to kick up and he fell with the ladder on top of him. Employee #1 was treated for a broken lower elbow and shoulder replacement and was hospitalized.
