Forklift rolls over employee's foot

At approximately 9:45 a.m. on January 6, 1998, former Employee #1, a former stock and inventory clerk for Michael Lisa Trucking Aka Sierra Alta Express, was using a forklift to move stock. The forklift rolled over former Employee #1's right foot possibly resulting in contusions or abrasions to his foot or breaking his ankle. Former Employee #1 was not hospitalized. On January 9, 1998, the forklift, 'Clark' Forklift Number 3 , Model Number JCS20 S/N 138MC was taken out of service for repairs. Former Employee #1 had not had the Employer's authority to move the stock.
