Investigation Summary
At approximately 9:00 a.m. on April 28, 1998, Employee #1, a plumber's helper, was on a portable ladder helping to raise a plumber's snake machine to a 12.5-ft-high roof. A coworker was on the roof pulling the plumber's snake onto the roof. The unsecured ladder slipped and fell together with Employee #1. Employee #1 was hospitalized for three days and was believed to have had cranial surgery.
