Investigation Summary
On July 9, 2004, Employee #1 was supervising a four-man crew engaged in electrical work at a home in Gardena, CA. He was coming down a ladder and was two rungs from the bottom when he bent over to pick up some tools on the ground. The ladder slid to the side and he fell, fracturing his left leg just above and below the knee. Employee #1's leg was placed in a cast and he returned to work the following day. The ladder was unsecured.
