Employee injures finger in punch press

On December 27, 2001, Employee #1 of Brack Manufacturing Co., Inc., was fabricating 2 in. diameter circular cold metal parts using a Part Revolution Minster Press (Model Number 6, Serial Number 6-10025, Type: geared, Tonnage: 60, Strokes per min: 50 to 100, Electric Power: 220-Volt 3-phase). The equipment was set on single stroke mode. Two hand controls were utilized to initiate every stroke. Parts were manually fed and extracted from the die area. When Employee #1 reached with his right hand to remove a formed metal piece from the lower die as usual, the ram of the machine came down and caught two of his right fingers at the pinch point. The company purchased this press from the manufacturer in 1955. Employee #1 was hospitalized for an amputated middle finger and a crushed index finger. The cause of the accident was lack of safeguarding the point of operation, which allowed employees to feed or remove parts with their hands at the point of operation.
