Investigation Summary
At approximately 9:30 a.m. on March 28, 2003, Employee #1 and another employee were assigned by the owner of the company to go to a salvage yard that was owned by another employer to get some parts. As Employee #1 was walking through the yard, he stepped on some discarded metal material and fell, breaking his right hip. He was taken to a local hospital, where he stayed for five days for surgery.
