Investigation Summary
At approximately 10:25 a.m. on November 20, 2007, Employee #1 was supervising the Packing Department, including the operation of a flow wrapper packaging machine, Model Number PFM 30 (PFM), the PFM Packaging Machinery Corp, identified by the employer as 1056. When wrapping material began to build up in the area around the transverse sealing crimpers of the PFM, Employee #1 went to clear the material with her right hand. She neither stopped the packaging machine, nor made sure that the power source was deenergized or disengaged. Employee #1's right hand was caught in the point of operation as the packaging machine cycled, causing the amputation of the distal phalanx of her second and third digits. Employee #1 was taken by ambulance to Loma Linda University Medical Center for treatment.
