Investigation Summary
At approximately 10:30 a.m. on October 24, 2003, Employee #1 was removing a wallboard in the unit that the employer was remodeling. This was a routine duty for the employee and he was working from a Werner 8-ft fiberglass A-frame ladder, Model FS108. The employee was on the top cap of the ladder, when an air conditioning duct immediately adjacent to where he was working struck the ladder. This knocked both the ladder and the employee to the floor. He was transported to USC University Hospital by the paramedics where he was admitted for surgery. The employee sustained a laceration to the scalp, a blood clot on the right side of the head, a fractured shoulder, two fractured ribs, and a fractured thumb. Employee #1 was hospitalized.
