Investigation Summary
A approximately 10:15 a.m. on October 24, 2003, Employee #1, a ground man, was climbing an extension ladder to the roof of the two-story building, a duty which he is not assigned to do. The employee stepped off of the ladder and onto the roof. As he was stepping onto the roof, the shingles of the roof gave way and the employee fell to the ground. The employee was transported to Cedars-Sinai Medical Center by the paramedics where he was admitted for surgery. Employee #1 sustained a laceration to the head and a cranial hematoma and was hospitalized.
