Employee injured after falling from scaffold

On March 20, 2002, Employee #1 and a coworker of Tenant Services Inc. were dismantling a scaffold. The scaffold was two frames high, approximately 15 ft and was not tied to the building. As Employee #1 was handing down some parts he leaned down away from the building and the scaffold tipped over. As the scaffold fell Employee #1 landed on the sidewalk, but the scaffold hit him on the back of head, dislocated his shoulder, and cracked three ribs. He was hospitalized for dislocated shoulder.
