Employee Suffers Amputation

Employee #1 was employed as an outside repairman at a company that manufactures and repairs ornamental iron gates, fences, and other decorative steel for architectural use. On February 13, 2002, Employee #1 was dispatched to repair an electric gate. While replacing the fence starter belt the gate unexpectedly started and the tip of Employee #1's left index finger was injured to the extent that it had to be amputated to the first joint. This gate could be operated remotely and lockout procedures were not employed. Employee #1 was hospitalized.
