Employee Injured In Fall From Ladder

At approximately 12:00 p.m. on March 3, 2000, Employee #1 and a coworker were assigned to stock rolls of fabric on metal racks. The fabric was in rolls weighing approximately 25 pounds each. The coworker was climbing the portable ladder stand with a roll of fabric on his shoulder and Employee #1 was above him receiving the roll. Employee #1 was piling the rolls of clothing materials at the top-most rack, about 12 ft above the floor. Employee #1 was seated on rolls of materials with one foot on the top step of the ladder. Employee #1 had no fall protection of any kind. The rolls of material Employee #1 was seated on moved and he lost his balance and fell 12 ft to the concrete floor. Employee #1 suffered head trauma and was hospitalized.
