Investigation Summary
At 3:00 a.m. on October 25, 1999, Employee #1, a warehouse man stocking merchandise, was assigned to load and unload merchandise on trailers parked with their backs on a series of loading docks. At the time of accident, Employee #1 was working on loading dock Number 41. He stepped over onto a trailer bumper which gave way, causing him to fall over 4 feet to the concrete pavement below. Employee #1 sustained multiple fractures of his right leg, and was hospitalized.
