Investigation Summary
At approximately 1:30 p.m. on October 21, 1996, Employee #1, of Gatea Painting, was scraping paint from the exterior of a single-family residence. He was standing at the middle of a 12 ft extension ladder when it slid to one side, causing him to fall. Employee #1 suffered a broken arm, and was transported to Cedars Sinai Medical Center for treatment. The ladder was not secured against displacement.
