Investigation Summary
Employee #1 was on a 20 ft extension ladder, using a battery-operated drill to replace aluminum screens. He was pushing on a screen to drill holes for securing the screws when the ladder slid out at its base and overturned. Employee #1 fell approximately 15 ft, striking an air conditioning unit approximately 4 ft off the ground. He fractured the lower part of his right leg as he landed. Employee #1 was transported by paramedics to the UCLA Medical Center. The ladder was not secured and it had been placed at an angle greater than 1:4.
