Investigation Summary
At 1:00 p.m. on August 25, 2017, Employee #1 was walking down the aisle, on the opposite side of the labeling machine. The employee noticed that the labeling machine had become jammed, and reached in while it was running. The employee's finger came in contact with the ingoing nip point and amputated his finger tip.
