Employee's finger crushed by a hammer

On April 21, 2003, Employee #1 and his supervisors, of Cascade Die Casting Group, Inc. were removing an angle-bar out of a mold with a ram hammer. The hammer was approximately 40 pounds, and it originally had two handles attached on either side of the hammer, but one of them had been broken off. While he was holding the hammer with hands and hammering, his left hand slipped due to his inability to firmly grip the hammer because of the missing handle. This resulted in hammer to striking his left index finger. He was hospitalized and the crushed part of the finger was amputated in a surgery.
