Investigation Summary
At approximately 11:00 a.m. on March 18, 2003, Employee #1, a machine operator for an employer that manufactured motorcycle parts, was operating a horizontal swedge which formed or bent-in the end of the metal pipes to smooth the edge. The machine had two settings, jog and production mode. The jog mode was only to be used to set up the machine, but employee #1 was making the parts in jog mode rather than in production mode. While loading a metal pipe on the machine by holding it by the edge, he accidentally pressed the control button, which caused the plunger (or ram) to move and trap his hand between the pipe and the die. Two hand-held controls were only operable in production mode and there was no device or equipment that prevented the employee from accidentally reaching into the point of operation in jog mode. The employee's right hand was crushed and two fingers were later partially amputated. The employee was hospitalized with his injuries.
