Investigation Summary
On July 11, 2003, two side-loader type vehicles emptied refuse at a municipal landfill. Their trucks were parked along the side of the entrance road, one in front of the other. The trucks were facing downhill on a two to three degree grade. Employee #1 was the driver in the rear vehicle, and he had left his truck with the engine running to speak with the driver of the front vehicle. The coworker stated that he felt a bump from behind while he was seated in his vehicle, and when he got out of his vehicle to investigate, he found Employee #1 wedged between the two trucks. The coworker stated that he ran to the rear truck to back it up. He noticed that the engine was running and that the brakes had not been set. Employee #1 was pinned between the two trucks and was crushed to death.
