Employee amputates thumb and finger in a table saw

At approximately 1:30 p.m. on October 10, 2003, Employee #1, a mill worker, was told to dispose of a wooden pallet. Because the pallets were too long to fit in the trash bin, he had to be cut into two pieces to be disposed. Often the pallet cutting was done with two employees, one holding the end of 84.5 in. by 36.5 in. pallet and the other employee standing near the middle of the pallet to cut it with a table saw, Powermatic with the Model Number 72 and Serial Number 3-157. On the day of the injury, Employee #1 was trying to cut it by himself because the number of employees was low that day to get help. At that time, he may have placed the guard in up position before cutting or the impact created from in-feeding the pallet may have lifted the guard. Once the guard was lifted, it was made to stay locked in up position, which exposed the blade. While Employee #1 was cutting, he experienced the pallet kicking back, due to which his hand touched revolving blade. He severed his right thumb, index and middle fingers. He was hospitalized and treated for amputation.
