Investigation Summary

At approximately 1:30 p.m. on May 5, 2003, Employee #1 was assisting another employee to diagnose a take-out table assembly that was not properly seated. The part had been extracted from its production line application and was in the maintenance shop on a hand operated overhead hoist. The unit was secured to the chain of the hoist with an eyebolt screwed into the top of the unit. Employee #1 had his hands on top of the unit to steady it while another employee was examining the assembly from below. The seat of the take-out table at some point shifted causing the unit to drop, trapping Employee #1's fingers. Examination of the hoist at the time of the inspection did not reveal any defects in the hoist. No signs of stripped threads or damage were observed at the top of the unit's eyebolt assembly. Employee #1 sustained three broken fingers to the right hand and was hospitalized.
