Investigation Summary
On April 20, 2003, Employee #1, an employee of Davis Wire Corp., 5555 Irwindale Ave, Irwindale, CA, was in the process of replacing a starter in a Hyster industrial lift truck. Employee #1 used a gray walking beam with an 8000-lb-capacity hydraulic fork lift jack, model number flj-400a, to elevate the back of the Hyster industrial lift truck that weighed 9800 lb. Since the forks were lowered and the front wheels were resting on the ground the jack did not lift the entire weight of the fork lift. Another employee was working on another fork lift about 20 feet away when he heard a noise. He turned toward the noise and saw Employee #1 under the fork lift and a stream of blood. Per the second employee, the jack was elevated and was a few inches behind but no longer under the fork lift. He said the right rear wheel was resting on a block of wood. This block of wood consisted of two, 2-in. by 6-in. boards nailed together. Another similar board was lying near by. Employee #1 was transported to Queen of the Valley Hospital where he was pronounced dead of a concussion.
