Investigation Summary
At approximately 8:00 p.m. on March 17, 2003, Employee #1, an employee of Total Warehousing, Inc., was repairing a Smetco sort conveyor pallet stacker, Serial Number PSU40E-1099-CHNY-4. The stacker, which was activated by a light beam, was stacking pallets in an uneven manner. While repairing the stacker, the employee inadvertently activated the light beam. This caused the lifter to raise and crush the employee's head and face, killing him. The employee did not deenergize or lockout the machine before beginning the repair.
