Investigation Summary
At approximately 8:45 a.m. on July 16, 2002, Employees #1 and #2 were working together on the scaffold performing a routine job, laying block on a scaffold with outriggers at a height of approximately 6 ft, 8 inches when it became unbalanced, possible by the weight of the two employees moving toward the block. Employee #1 lost his balance first and fell. He sustained a laceration on the left elbow which required surgery and a hospital stay in excess of 24 hours. Employee #2 fell a moment later. He did not sustain a serious injury. He was treated, observed, and released within 8 hours. Both men were taken to Pomona Valley Medical Center by ambulance for treatment and observation.
