Investigation Summary
At approximately 1:30 p.m. on February 8, 2002, Employee #1 was replacing pallet-braking steel tracks on a storage rack. The employee was lowered by a forklift from the top shelf of the rack to the floor and was standing on a pallet (platform) on the forks of the lift when he lost his balance and fell. The employee suffered a fractured left ankle and elbow requiring a 39-day stay in the hospital.
