Employee's index finger amputated in saw accident

At approximately 7:30 p.m. November 2, 2001, Employee #1's left index finger was amputated when he lost control of a radial arm saw. Employee #1 was employed in the lumber department of a Home Depot at 9200 Whittier Blvd. in Pico Rivera. While he was cutting a 2 by 4 board, the saw's retracting cable broke, he let go of the handle used to pull the blade across the wood and lost control of the saw. The wood seemed to twist into the blade carrying the employee's left hand with it and amputating his index finger. The equipment was well-maintained, the employee trained, the saw inspected weekly, and no fraying of the cable had been observed. All required guards were in place.
