Investigation Summary
At 12:45 p.m. on May 24, 2001, Employee #1, a machine operator employed by Pabco Paper in Vernon, California, sustained a crush injury to his left hand as a result of an accident. Employee #1 has been an employee of Pabco Paper for about 8 years and was about to be promoted to third hand operator prior to the accident. Employee #1 was working alone with four other crew members on a machine that manufactured drywall paper. The accident occurred at the fourth dryer section of the dry end. Employee #1 was instructed to adjust the paper that was passing through in a running roller with his hand. The job required operators to use their hands to help stretch the paper so that the paper would stay on track in the rollers. The leadsman of the crew instructed Employee #1 to assist the third hand operator by using his hand to stretch out the paper so the paper could stay within the rollers. While Employee #1 used his hand to do what he was told, his left hand was pulled into the in-running rollers. Employee #1 was wearing gloves at the time of the accident because of the high temperature of the paper been manufactured. There were no guarding devices or barrier guards available at the time of the accident to protect the operator's hand from the pinch point of the running rollers. According to the employer's internal accident report, Employee #1 was told by the leadsman to hit the quick draw on the fourth section. Instead, Employee #1 misunderstood and tried to pick up the slack by hand over Dryer 420 and his hand was taken in by the roller. The employer blamed the lack of communication between operators and believes that better communication needs to be enforced to prevent any future reoccurrence. Following the accident Emergency Medical Services was called and paramedics responded. Employee #1 was taken to County USC Medical Center and was later transfer to the Western Hand Center. Three of Employee #1's fingers on his left hand were amputated as the result of the accident.
