Employee's fingers amputated in table saw

At approximately 9:45 a.m. on June 6, 2000, Employee #1, of Cervitor Kitchens, Inc., was using a table saw in the cabinet shop to make a special groove on a 2 in. by 3/4 in. by 1/2 in. wood block. His left hand struck against the saw blade, amputating portions of his thumb and of his index and middle fingers. Employee #1 later stated that he had raised the guard into an upward position for the job, and that he should not have been feeding in the block from the back of the saw. He also admitted that he had not been attentive during the operation.
